Required Forms and Attachments

To complete the commission appointment process, HM Insurance Group requests the completion of the forms and supporting documents listed below. E-signature, a secure and convenient way to provide information and signature, is now available (no need to print and scan paperwork). When completing the e-signature, a copy of the signed form will be emailed to you.

1. Individual Appointment Request Form (Copy of Producer License is required)

Instructions: Fill out the form, attach a copy of your producer license and sign electronically to complete.

2. Certifications and Authorization to Obtain Investigative Consumer Report
